If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out before we can treat you.  Printing and filling them out to bring with you at your first appointment means we can get right to your dental needs.  Otherwise, please come 15-20 minutes before your appointment so you have enough time to fill them out.
